Unity Health Insurance:
50% off Fit Fun Bootcamp Program up to $130. This is a one-time rebate, so to get the full amount, you would need to attend at least 2-3 months worth of bootcamps and have the receipts.

Physicians Plus Insurance:
$100 rebate, up to $200 per family for participating in Fit Fun Bootcamps. All you need to do is download the the form below and mail it in with your receipt.
Physicians Plus Reimbursement Form

GHC Insurance:
$100 individual rebate, up to $200 per family. You must attend 120 session in a year and keep track of them in a log. Once you have the required number of sessions, click the link and you will be sent to a form to be filled out. Along with the form you need a copy of your Fit Fun Bootcamp receipt which you receive immediately after you pay online. For questions call 608 251-3356.
WPS Insurance:
Unfortunately, WPS is behind everyone else and doesn’t offer any sort of money discount to its customers. They said they hope to in the near future. If you have insurance with them, I would contact them and tell them that you would like to see WPS include some sort of money back rebate for health-based programs. Blue Cross Blue Shield:
BCBS give NO health incentive programs of any kind to their customers. If you are unhappy with that, feel free to contact them at 1-888-641-5224.
